He has very poor social support and is seeking treatment for the first time today despite being diagnosed several months ago. His reluctance to seek treatment stem from both economic concerns and from social concerns. Addressing these social and economic concerns will be a vital part of his treatment plan as they are currently barriers to him receiving the consistent medical care he needs. There is a wealth of literature related to impacts of social and economic barriers on treatment retention and something as simple as not disclosing HIV status to friends and family or lack of support from those whom the patient has disclosed their status to can significantly decrease the adherence to treatment regimens (Arnold, 2017). The primary nursing diagnosis is risk for infection due to compromised immune function. Secondary infections, including fungal infections are a significant concern for HIV patients. Due to this increased risk routine monitoring of CD4 counts is recommended along with prophylactic antifungal medications when CD4 counts are below threshhold (Limper, 2017). The primary desired outcome related to this nursing diagnosis is remaining free of secondary infections and complications.
