NR 630 Initiation of Capstone Project- Staffing Shortages Raul Guardiola Chamberlain College of Nursing Executive Practicum NR 630 Dr. Patricia Hampton November 25, 2017 Initiation of Capst... one Project- Staffing Although recent growth in both registered nurse (RN) employment and nursing school enrollment suggest potential easing of hospital nurse shortages, nurse staffing will likely remain an important issue for health care providers in coming years. “Studies forecast a growing gap between nurse supply and demand, with some estimates suggesting a deficit of more than 900,000 nurses by 2030,” (Standford, 2013, p. 42). A comprehensive, strategic workforce planning approach focused on identifying the role that are essential for current and future success is fundamental. Strategic workforce planning requires that system leaders have a vision of how they will deliver care in the future. Human resource, finance, strategy, and clinical leaders should work together to project a year-by-year forecast of what the integrated delivery system may look like. By identifying strategic roles for the future, we can forecast and profile the available and required talent pool within and external to an organization. Therefore, the purpose of this paper is to introduce the authors capstone project on how staffing shortages will impact patient care. The following is an annotated bibliography which summarizes and evaluates ten sources that address the nursing shortages. Summary: The article offers information on the study conducted by researchers from the British National Audit Office (NAO) which found that delays of patient discharge in the health care industry of the country related to the 15% staffing shortage in home care and nursing facilities. The article provides reasonable and interesting solutions to the nursing shortage. The article is current and will be used in the completed paper on the nursing shortage. Brahm, P. (2016). Staffing shortages can fuel increased discharge delays. McKnight’s long-term Care News, 37(7), 10. Summary: Staffing shortages among professionals in specialized health care and clinical situations that demand around-the-clock care make it a challenge to address staff member fatigue. Many industries have developed restrictions on work hours to help reduce the effects of fatigue on safety. The perioperative specialty, however, often requires personnel to work long hours, offers limited resources for specialized professionals, and employs on-call schedules that can be demanding. This article discusses research and additional information related to fatigue and how it can affect perioperative patient care. The article is current and will be used in the completed paper on the nursing shortage. Battie, R. N., Rall, H., Khorsand, L., & Hill, J. (2017). Addressing perioperative staff member fatigue. Association of Operating Room Nurses Journal, 105(3), 285-291. Summary: Surgical nursing requires specialized training and skills that are not routinely offered as part of the curricula in most nursing schools. Instead, the extent of many nursing students’ exposure to perioperative nursing is limited to the occasional observation assignment, which prevents them from comprehending the role of circulating, scrubbing, or first assisting during surgical procedures. This creates a knowledge gap in clinical nursing education. As a result, nurse graduates are not aware of the full scope of the perioperative specialty and therefore do not apply for perioperative positions. The limited number of nursing school faculty members with perioperative experience also contributes to the lack of perioperative nursing education in academia. This article discusses research and additional information related to the prediction of worldwide perioperative nursing shortages. Ball, K., Doyle, D., & Oocumma, N. (2015). Nursing shortages in the OR: Solutions for new models of education. AORN Journal, 101(1), 115-136. https://doi.org/dx.doi.org.chamberlainuniversity.idm.oclc.org/10.1016/j.aorn.2014.03.015 Summary: This paper explores the mismatch between the challenges faced by nursing graduates (of whom many belong to Generation Y) in obtaining a Registered Nurse (RN) position following graduation and the continuing shortages in nurse workforce in aged care and mental health areas. The article is current and will be used in the completed paper on the nursing shortage. Christopher, S. A. (2016). Can generation Y nurses supply areas of shortage? New graduate challenges in today’s job market. Australian Journal of Advanced Nursing, 33(2), 35-44. Summary: This paper discusses the nursing shortage and present a unique preceptorship program that was developed to address the perioperative nursing shortage at three healthcare institutions. The program involved collaboration between the hospitals that were experiencing high attrition rates and a university located in the State of Michigan in midwestern United States. This paper discusses the first perioperative class that was offered. The article is current and will be used in the completed paper on the nursing shortage. Carter, M. J. (2015). A student nurse experience of an intervention that addresses the perioperative nursing shortage. Journal of Perioperative Practice, 25(11), 230-234. Summary: The author discusses the challenges in recruiting nurses as to rules from Government regulators including the Nursing and Midwifery Council, the Care Quality Commission (CQC) and Monitor. She agrees that extended services of many trusts require additional nursing staff to provide needed services. Also discussed are the Department of Health (DH) announcement about a cap on spending by setting a ceiling hourly rate for nurses and doctors and banning the use of non-Government-approved frameworks. Foster, S. (2015). Challenges to unfilled posts and nurse recruitment. British Journal of Nursing, 24(18), 937. https://doi.org/dx.doi.org.chamberlainuniversity.idm.oclc.org/10.12968/bjon.2015.24.18.937 Summary: The article reports on a study which found that a failure to complete nursing care explains why hospitals with lower registered nursing staff levels have a higher risk of patient death. These results give the clearest indication yet that RN staffing levels are not just associated with patient mortality, but that the relationship may be causal. If there are not enough registered nurses on hospital wards, necessary care is left undone, and people’s lives are put at risk. The article is current and will be used in the completed paper on the nursing shortage. Missing link is shortage of RNs to provide care. (2017). Retrieved from anmf.org.au Summary: The article discusses how innovative information technology solutions can address nurse shortages and staffing challenges. Topics covered include the need for healthcare systems to support the role of chief nursing informatics officer and nurse informatics leaders, the estimated shortage of nurses and midwives in 2013 according to the World Health Organization, and the use of technology and informatics to drive decision-making and policy development processes. Lloyd, J. (2017). Innovative information technology solutions: Addressing current and emerging nurse shortages and staffing challenges worldwide. Nursing Economics, 35(4), 211- 212. Summary: The article reports on the incentives being offered by long-term care providers in the U.S. as of August 2016 to bring more nurses to their facilities due to increasing nursing demand, aging population and rising incidence of chronic conditions. The article is current and will be used in the completed paper on the nursing shortage. Gullatte, M. (2016). Nursing workforce shortage. Georgia Nursing, 76(3), 10-10. Summary: The article reports on the emergence of licensure compacts in a move toward the implementation of multistate license, which allow nurses in the U.S. to practice their profession in another state without obtaining additional license as of July 2016. Topics discussed include signing of a legislation in Arizona which allows a nurse to travel and work in a partnering state holding just a single license, and possible drawbacks of multistate licensure such as loss of revenue from licensing fees.
