NURS 4581 Critical Care - Exam 1 Notes, University of Texas. When a metabolic acidosis occurs, the cell acts as a buffer by moving the excess H+ ions into the cell. In exchange, the cell will “kic... k” out potassium. With polyuria, the patient has “lost” K+, causing a decrease in the serum K+. As the sodium bicarbonate (NaHC03) is infusing, the pH increases. Hydrogen ions will move out of the cell. As H ions exit the cell, K ions will go back into the cell. The patient’s K+ level will decrease even more and the patient can go into an abnormal cardiac rhythm. Clinical note-if your patient is acidotic and HYPOkalemic-be cautious before correcting/treating the acidosis.
