As a health information management professional, I gave been asked to review and determine the HCPCS II codes for the medical record of an outpatient patient named Lonnie Lates. Lates, is a 64-year-ol... d female who is diagnosed with Parkinson’s disease, and was ruled out from cerebral vascular insufficiencies. In the duration of her three-day hospital admission she completed a neurological exam, completed blood count done, and utilized an ambulance service. She was also administered the following medications: Symmetrel 100 milligrams, Halcion .25 milligrams, Metamucil one tablespoon, Enema, Klotrix, Colace 100 milligrams, Restoril 15 milligrams, Tylenol, Milk of Magnesia, Meprobamate 400 milligrams, and Erythromycin 500 milligrams. This information was concluded by the review of her entire medical record. The main term for Lates’ neurological exam is neurological. As for her complete blood count, the main terms are CBC and complete. The main term for her utilization of the ambulance services is ambulance. Lastly, each name of the medication she was administered represent themselves as a main term. These terms were determined while researching through encoder, in while aided me in locating all relevant codes related to the term. Starting with the neurological examination, I used the selectcoder search bar, typed in neurological, and clicked HCPCS filter. This action allowed me to choose from two HCPCS codes: G8404 and G8405. I repeated these steps for the next two procedures. After using the main term CBC (complete blood count) I was given to options for this as well: G0306 and G0307. Lastly, after my third search for the ambulance services using the main term ambulance, I received 20 codes as a result. This led me to add an addition search word as service, in which it narrowed my selection to the following codes: S9960, S9961, A0021, A0225-A0431, A0999, A0384, and A0392. As stated before, Lonnie Lates was administered multiple medication in which I used the medications name as the main term. Upon searching for the term Symmetrel I was unable to gain any information, which led me to find alternative names for this medication, Amantadine Hydrochloride. After using Amantadine Hydrochloride in selectcoder the two codes I was given was G9017 and G9033. Halcion also did not produce any results; therefore, I used its alternative name Triazolam. However, upon looking for Triazolam in its HCPCS codes I also could not produce any results.
