Case Scenario: Answered step-by-step Mrs Stella Harris is a 72-year-old woman with a past medical history of osteoporosis, and ischaemic heart disease. Stella lives alone in an aged care unit, with... nursing care support from Home and Community Care Services. She is usually mobile at home with her wheelie frame, and uses her gopher to go the shops and other places within the neighbourhood. Her daughter Michaela lives 5 minutes' drive away and regularly visits her mum. Stella's other children also provide needed support but live further away. Her husband died four years ago from cancer. Stella cared for him at home before he died. Stella has recently been feeling quite frail, with increased lower back pain. She is admitted to your ward for supporting nursing care while having a series of tests and procedures. Whilst showering Stella, the nurse caring for her briefly left her on her own to get a towel. Stella slipped on the wet floor when trying to stand up from the shower chair and suffered a fall in the shower. The doctor has assessed Stella following the fall. She complains of pain in her leg, and x-rays have revealed a fractured neck of femur. The doctor has advised the staff to administer the prescribed pain relief. Stella has also been booked to go to theatre this afternoon to treat the fracture surgically. She will also require at least 2-3 weeks rehabilitation before she can return home with community support. Stella's family are now in the hospital and expressing anger about the standard of care provided for their mum, her need to now have surgery, and the extra pain she is experiencing. Describe the process of open disclosure in details: 1. Revise the adverse event that has occurred and provide opportunity for staff members to contribute any feedback or personal responses to this event. 2.
