Guided Reflection Questions Opening Questions How did the simulated experience in Lucy Grey’s case make you feel? This simulation made me feel confident in my ability to perform an ED triage. I... felt I was able to make the correct assessments and ask the right questions to obtain an accurate picture of what was going on with Lucy in this specific simulation. What nursing actions did you feel were appropriate within this scenario? Washing my hands, identifying the patient, assessing the patients vital signs, assessing the patients skin and wound, performing a head to toe assessment, and completing the Morse Fall Scale assessment tool. I also provided patent and family education on family involvement, coping with anxiety, and available resources for Lucy. Scenario Analysis Questions* PCC/S What priority problem(s) did you identify for Lucy Grey? Lucy’s priority problem this visit to the ED was her dizzy spell and near fall. After communicating therapeutically with Lucy, I was able to determine that she has been struggling with anxiety and depression after the loss of her partner to lung cancer. PCC How might the death of a spouse or significant other impact the biological and psychological well-being of the surviving partner? The death of a spouse can affect the well-being of the surviving partner is many ways such as depression, grief can have physical consequences such as sleeplessness and loss of appetite. For the elderly, bereavement can have a devastating effect on their immune system and cause them to lose interest in their own care. The loss of a partner can result in loss of independence, new responsibilities, and isolation on top of all the grief of losing a loved one. PCC What strategies could be implemented to address physical and psychosocial problems associated with Lucy Grey’s ineffective coping skills in the loss of Julia Morales? I would ensure that Lucy is safe and capable of managing her own care at home, and if not help her to establish the assistance she needs whether that be from her son in law Neil or visiting healthcare personnel. Try to help her learn how to manage new tasks around the home that used to be completed by Julia. Encourage her to set up daily phone calls with her son in law Neil to help her feel less isolated. Encourage her to developing healthy coping mechanisms such as deep breathing, vivid imagery, etc. I would also like to put her in contact with a mental health consular where she can discuss her anxiety and depression in a more detailed manner, EBP What potential problems does Lucy Grey face if her symptoms are not recognized and if actions are not initiated in a timely manner? My biggest concern is that Lucy could possibly die at home if her symptoms are not recognized and if actions are not initiated in a timely manner. She is now living a lone and if she becomes unable to ambulate or call for help quickly she could be in grave danger. PCC/I What appropriate assessments were made to help you decide what interventions Lucy Grey needed? A Morse fall risk assessment was made and it was determined that Lucy is at high risk for falls. This helped me decide that Lucy should be seen by a physical therapist to determine what assistive device would be best for her to use at home to decrease her risk of falls. S What safety measures should Lucy Grey and her niece take to decrease Lucy’s risk for injuring herself from a fall in the future? Lucy should remove any fall hazards from her home if possible. This includes area rugs/carpets, extension cords, small tables, and clutter around the house, especially on the floor. She should also begin to use an assistive device such as a cane or walker at home. Concluding Questions Reflecting on Lucy Grey’s case, were there any actions you would do differently? If so, what were these actions, and why would you do them differently? One action I would change about the care I provided to Lucy would be to perform a geriatric depression scale assessment. This is something that I forgot to complete in my initial care for Lucy, and it is very critical in determining the level of behavioral health care that she requires. How would you apply the knowledge and skills that you obtained in Lucy Grey’s case to an actual patient care situation? If this was a real life patient, I would take these triage skills that I learned and apply them very thoroughly. I would take my time to ensure I am performing all of the necessary assessments, skills, and questions in order to obtain an accurate picture of Lucy’s case.
