Opening Questions How did the simulated experience of Eva Madison’s case make you feel? This wasn’t at all what I expected. I had to redo the simulation because I didn’t quite understand wha... t I was supposed to do. I very much enjoyed it though once I had a grasp on things. I did not expect this to be so life like. This is what I would imagine being in a clinical situation with a child would be like. Describe the actions you felt went well in this scenario. Once I figured out what I was supposed to do, I jumped right into assessment. As a nurse, you are always assessing. I directed my assessment primarily to the systems and functions related to her symptoms. The most important parts of an assessment to me in Eva’s case would have been vitals, pain assessment, and GI assessment. I Know I don’t know nearly enough but the one thing I feel comfortable doing is assessment. Scenario Analysis Questions1 EBP What priority problem did you identify for Eva Madison? Dehydration and pain EBP What complications might Eva Madison face if her symptoms are not recognized and actions are not initiated in a timely manner? Dehydration. She was not seriously dehydrated however, she began to show signs such as dry and cracked lips, stating she was thirsty, and her HR began to elevate. If her dehydration had continued to get worse, her blood pressure would go down and her electrolyte balances would be at very low and dangerous levels. Potassium for example is very important for heart function PCC What actions should be implemented to decrease anxiety in Eva Madison and her mother? Assurance. The nurse explained to both Eva and her mom that she had gastroenteritis but assured her she would be okay. One of the nursing interventions was to provide Eva with a stuffed animal. Items like this can help provide comfort. It is important to include both Eva and her mother into Eva’s care. They should both be aware of everything that is going on and not be left in the dark. Any diagnostic testing or medication administration should be presented in a easy to understand and non-scary way. PCC/S What should you teach Eva Madison and her mother about home care of a patient with gastroenteritis? Remain on a clear liquid diet for the next 24 hours. Avoid sweet carbonated and caffeinated drinks. If vomiting or diarrhea persists or worsens return to doctor/ER. Watch for signs of dehydration such as thirst, dry-cracked lips, dark colored urine, sleepiness, low energy. T&C What role do you feel Eva Madison’s mother has as a member of her health care team? (Explain your answer.) Eva feels more comfortable with her mother than any nurse she will encounter. She is more likely you cooperate when her mother is there. She is a very important member of Eva’s healthcare team because she will be the one who continues Eva’s care once they go home. Education is an important part of the Care plan to ensure that Eva’s mother can provide accurate and beneficial care. T&C/I What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format. 5 yr old Eva Madison was brought in by her mother for vomiting and diarrhea for the past 2 days. Upon assessment I noted hyperactive bowel sounds. Eva stated her pain was at a 1 on faces (1-5) pain scale. Her HR was 111, Pulse Ox 97%, Temp-100F, and BP- 130/80. A stool sample was taken for culture and Health care provider prescribed 5mL of OHS every five min x2, and then 30mL every 15 min as tolerated. Education was provided to the patient and the patients mother on continued care at home and signs of dehydration. Concluding Questions Reflecting on Eva Madison’s case, were there any actions you would do differently? If so, what were these actions, and why would you do them differently? I kind of felt like I was all over the place. There wasn’t really much organization to how I did things. If I could change one thing, I would have done things in a more organized manner. Describe how you would apply the knowledge and skills that you obtained in Eva Madison’s case to an actual patient care situation. Although I’ve been to clinicals before, this gave me a better idea of how to act and what to expect as a nurse without someone standing over my shoulder. I wasn’t scared to screw up because its just a simulation. This gives me more confidence in interacting with patients in the real world. This made me realize that assessment is everything. You assess every time you walk into the room, give a new medication, or implement a new intervention. Whenever I implemented a new intervention whether it be pharmalogical or nonpharmalogical, I waited to see if there was a change.
