She is a single mother and the sole source of financial support. Jackson has not seen his neurologist in 15 months due to his mother's work schedule. Jackson was seen by his health care provider this morning and currently has an intravenous infusion in his left arm running D5 NS + 20 mEq KCl/L at 58 mL/hr. He has been eating well, has urine output, and vital signs are stable. His pupils are equal and reactive to light. He is able to move all extremities equally. He is awake and alert and is watching cartoons. His mother is at the bedside. Documentation assignment 1. Document your initial focused assessment of Jackson Weber. Initially the assessments I wanted to focus on were drowsiness, eating patterns, temperature, respiration, patient health history, and urine. I was on alert for loss of consciousness, aspiration, vasovagal syncope or fainting. 2. Identify and document key nursing diagnoses for Jackson Weber. Jackson has a history of seizures. Referring to your feedback log, document the nursing care you provided and Jackson Weber's respo The nursing care I provided was; ● Attached noninvasive blood pressure measurement cuff ● Looked for normal breathing , 14 breaths per min ● Assessed the IV , not complications ● Asked about past medications to parent, was being given phenobarbitals ● Patient began to have a seizure so I put him in a recovery position which as on his side. I also secures him to prevent injuries ● Communicated with the provider ● Check for responsiveness and brachial pulse ● Places a nasal oxygen cannula ● Gave 100% oxygen from a nonbreaking mask ● Checked pupils , they were 4mm and reactive ● Listened to the lungs, they were clear and equal bilaterally ● Assessed respiration, examined chest, 18 breaths per min ● Listened to the heart ● Examined mucous membranes ● Provided patient education before handoff 4. This stimulation had me scared when Jackson had a seizure because I had to take quick action through the computer but luckily the buttons were flashing so I knew where to click quickly, therefore I felt that the actions I took while and after the patient had the seizure went well. I placed the patient in the correct position and after the seizure I checked for responsiveness and focused on vitals. If so, what were these actions, and why would you do them differently? I would have assessed Jackson’s pain as an added assessment to what I already did, Jackson may have had pain before and after the seizure that was unknown. Also used my nursing knowledge to decipher if I should administer medication or not. I’m this case Jackson could not take his medication orally so the order could not be fulfilled. As a nurse your main duty is to protect the patients safety. Describe how you would apply the knowledge and skills you obtained in Jackson Weber's case to an actual patient care situation. I learned that during seizures the patient's safety is important so as a nurse if a patient is having a seizure I must use the hospital's seizure materials, such as if the hospital had seizure pads, and to clear any loose objects to maintain patient safety.
