E.B 16F S CC: Left ankle injury. HPI: This is a 16 year old female, who was brought in to the clinic by his mom after she injured here ankle. Elizabeth says that she is a dancer and reports pa... st injuries with her most recent injury 8 months ago, where she had lax ligament from groin pull. She reports that she underwent physical therapy (PT) evaluation and treatment and responded well to PT that she was cleared 3 weeks ago to return to dance without restrictions. Elizabeth reports that while at dance class last night , she landed on the side of left ankle and heard a pop. She says that she is unable to bear any weight as her left ankle gave away and unable to walk unassisted. She says she now requires crutches to get around. Mom says she just iced left ankle, and came here first thing in the morning. PMHx: Reports general health as good. No serious health issues. She had previous ankle and groin injuries related to dance. Childhood illnesses: Chicken pox Chronic illnesses: None Surgeries: Appendectomy 2 years ago Hospitalizations: None Immunizations: Childhood vaccinations UTD. Does not receive flu shot. Mother declined HPV series. Allergies: None Current medications: Daily multivitamin Social Hx: High school student in 10th grade. She has been in Irish dance for 8 years and has dance class twice a week. Denies ETOH, tobacco, illicit drugs. Sleeps 8 hours Family Hx: Parents are alive and well. Mother in good health and father has a history of thyroid storm 3 years ago. Brother age 12 in good health. Maternal and paternal grandparents are alive, paternal grandmother recently diagnosed with breast cancer, and paternal grandfather in assisted living due to dementia. [Show More]
