During this visit, Mr. Jones reports the onset of some new symptoms that he also would like to address. The purpose of the paper is to analyze the subjective and objective information provided to diagnose and a develop a management plan for Mr. Jones. This paper will discuss the assessment of all appropriate diagnosis and the different component such as medication, education, referral, follow up care, and the assessment of comorbidities and medication cost. This paper aims to used national and evidenced based guidelines to tailor a plan of care and education points that can address all of this patient medical needs and help control the different illnesses in the community. Assessment Mr. Jones presents to the clinic today for a planned six month follow up for hyperlipidemia and weight loss. The patient is reporting the onset of fatigue for the last ten weeks but the review of laboratory results reveals a TSH level and complete blood count level that are within normal limits therefore anemia and thyroid issues can be excluded. He reports having a hard time losing weight despite exercising. He also mentioned increased hunger and thirst with exercising and the need to urinate more often at night and during the day. Mr. Jones Hg A1c from the previous lab result was 6.9 with a fasting blood glucose of 135. The American Diabetes Association mentions that a diagnosis of Diabetes Mellitus can be established in a patient with a fasting glucose >125mg/dl or an Hg A1C >6.5%. Our patient meets the criteria for a diagnosis of DMII with her fasting blood results and her HgA1C. Mr. Jones lipid panel was elevated which confirms a diagnosis of hyperlipidemia which increases the patient cardiovascular risk especially with a diagnosis of diabetes. Patient has an history of Osteoarthritis that is well controlled with Tylenol and turmeric supplement. [Show More]
