Assessment Primary Diagnosis-Type 2 diabetes mellitus (ICD-10 code is E11.6) Pathophysiology- Type 2 diabetes mellitus is when the pancreas makes insulin to convert glucose into energy, but there is insulin resistance which caused a reduction of the use of insulin increasing blood glucose. The disorder is manifested by symptoms such as increased urination, thirst, hunger, fatigue, and dry mouth (Kahn, Cooper & Del Prato, 2014). pertinent positive findings- the patient has a glucose level of 130 mg/DL which normal ranges is between 70-100 mg/DL. The hemoglobin A1C level is 6.8% above the normal ranges of 4% to 5.6%. The signs and symptoms of diabetes are increased urination, tired feeling, hunger and thirsty (Inzucchi et al., 2015). pertinent negative findings- there no signs of weight loss rather the patient has gained weight.
