LEADERSHIP EXPERIENCE 2 A1. Problem or Issue The problem identified in this task is the lack of hospice referrals within my facility. This problem has been presented not only in my facility but in... many health care centers nationwide. Considerable research indicates that hospice care is underutilized despite its proven effectiveness for the care of patients who do get referred to this care. For a facility, it is essential to have the necessary resources to provide hospice care, as well as a protocol to refer patients who need it. Finally, it is crucial to analyze the existing barriers to access hospice care to find a practical solution. A1a. Explanation of Problem or Issue As a nurse in a Home Based Primary Care (HBPC) team, the geriatric population, the seriously ill population, and their needs are one of my most significant responsibilities. The lack of referrals to hospice care affects not only the facilities and my team but also the patients. Hospice care is used to provide care centered on the patient and not the disease. Patients with terminal illnesses and a limited time can be referred to hospice care, in which an interdisciplinary team will work together to provide the necessary care, respecting that person's choices about end-of-life care (McAteer & Wellbery, 2013). The lack of referrals to hospice care limits many patients' life expectancy and affects the costs of the facility. For many patients, hospice care is a financial relief as it is a costeffective alternative. When hospice care is not used, patients are more likely to go to the intensive care unit, and in general terms, their vital functions are considerably reduced (McAteer & Wellbery, 2013).
