Pain may radiate to R shoulder. Can lead to gangrene of gall bladder, needs hospitalization. 3. Diverticulosis Presence of diverticula without inflammation 4. Acute Diverticuli- tis 5. Diverticulitis dx/tx 6. Acute Pancreati- tis Pouch like herniations (diverticula) on external surface of colon become inflamed/infected. ‘elderly Complic. incl abscess, sepsis, ileus, bowel obs, hemor- rhage, perforation, fistula & stricture>>death. Acute onset high fever, anorexia, N/V, LLQ abdominal pain. Rebound tenderness, +Rovsing, board-like abdom PE w/+ Rovsing, acute abd, hematochezia/anemia CBC (leukocytosis, neutro>70% L shift FOBT (+ if bleeding) Reticulocytosis if acute bleed, low Hgb/Hct TX Cipro 500 BID + Metronidazole 500 TID x 10-14d Close follow up High fiber diet Inflammation of pancreas; pancreatic enzymes activate causing autodigestion. Adult c/o acute onset fever, N/V assoc w/rapid onset of ab- dominal pain that radiates to midback (boring)**, located in epigastric region. Causes include drugs, biliary comp, ‘triglycerides, infection and ETOH abuse. 7. Pancreatitis dx/tx Guarding/tenderness over epigastric area/upper abd Cullen's sign + (bluish discoloration around umbilicus) Grey-Turners sign + (bluish discoloration at flank) Hypoactive BS, jaundice, guarding, and board-like abd ‘pancreatic enzymes (amylase, lipase, trypsin) ‘AST, ALT, GCT, bili, leukocytosis...
