David Carter is a 28-year-old male with a 10-year history of schizophrenia who lives with his mother and works as a grocery clerk. Onset of schizophrenia was while David was in high school, with progressive social isolation and difficulties with school and work. His first psychiatric hospitalization was at the age of 22 years, when he presented with auditory hallucinations and paranoid delusions. He has been hospitalized twice since then and tried various medications. His current medications include olanzapine 10 mg and extended-release (XR) venlafaxine 75 mg daily, but he recently has stopped taking his medications because he has put on weight since he was shifted to the current medications about 1 year ago. His mother called the police earlier today after he became violent with her, and the police brought him to the hospital. In the hospital, he has vacillated between being withdrawn and agitated, believing that other people can hear his thoughts and that the hospital food is poisoned. His speech is disorganized and expresses themes of generalized paranoia. It is difficult to carry on a conversation with him. Students should complete a focused mental status assessment, assess the patient’s thought processes, paying special attention to the delusions and hallucinations, orient the patient to reality, monitor nutritional status, and maintain a safe environment for the patient and the nurses.
