Patient safety has been at the core of every health care organisation and every effort is made to enhance the quality of healthcare offered to patients. The 21st century has seen a revolution in the r... egulatory measures, as governments worldwide are encouraging organisations to take responsibility and put measures in place to enhance quality and safety. Furthermore, organisations and professional bodies have a legal and ethical responsibility to offer quality health care services to their patients. Nevertheless, it is also important to note that there may be failures in the system and the causes that contribute to the deteriorating quality of healthcare services which infringe on patient safety. However, this can help in finding effective solutions to problems that can help healthcare practitioners improve the quality and safety of patient care. Medical scandals show that healthcare practitioners cannot always be relied on to provide quality healthcare services even though they put the interest of the patients first (Sroczymski et al., 2012). While many people enjoy quality health care in this country, an alarming number of tragic events occur that is caused by healthcare practitioners. However, according to Sohn (2013), it is important to understand the concept of adverse events and negligence. For instance, if a patient diagnosed with pneumonia is treated with an antibiotic then complications from the drug arise this is not a case of negligence since negligence is the intentional failure to provide standard care to a patient (Sohn, 2013). According to a study by the Harvard Public Health, only 27% of cases reported are due to negligence (Sohn, 2013). This shows that errors occur in the practice of healthcare practitioners because a hospital is a complex organization in which patient care and safety incorporates many technical procedures undertaken by the practitioners working in a high- pressure environment. In this regard, regulation is an important aspect of healthcare and organisations need to embrace this to ensure quality and safety in the work place for patients. According to Skar (2010), regulators of nursing practices experience many challenges, which include the complexities of an aging population and changes in the delivery of mental health services. Its aim is to support and fosters a culture of continual learning one that ensures patients are always at the centre of care planning and delivery and where staff are supported to deliver care that is safe, effective and compassionate (HSE, 2016) Primary Care Making health care safe and quality involves everyone from policy makers, regulators, and health care practitioners. According to Sohn, (2013) regulation of the health care services involves the integration of multiple strategies. A single regulation cannot cover all the factors that influence nursing practice and have all the knowledge required to influence change in the complex health care system. Moreover, smart regulation entails a networked governance in which the different regulators come together and formulate mechanism so that the weakness of one regulator can be covered by the strength of the other regulators and vice versa. Furthermore, regulators should be flexible and try different approaches to ensure patient safety and quality health care services clear accountability is imperative (HSE,2012) The function of health care is to provide preventive, diagnostic and emergency care for patients who need care and organisations have evolved over the years. There are many important aspects of an excellent running health service: patient care, good providers, safety, personal responsibility, open culture, to name just to name a few. Many performance variables are there to identify the effectiveness of a service. The very first and important limitation is the satisfaction rate of the patients. Effectiveness of the service can be measured by the patient’s satisfaction of the health care services and the quality of the same being provided. Also, the dissatisfaction rate would show the opposite. This can be done by asking patients and their relatives to fill out a form (feedback) or a questionnaire like a survey. Another thing that can be done is by knowing the motivation and satisfaction level of employees who are working inside the service, as this can be directly linked to the quality of service and care provided to patients. There are many important decisions such as expansion, of activities and services, along with the approval of many financial plans, which should be completed carefully as they have been done ethically. There are also many factors to be considered when designing an expansion plan and if would be beneficial. The expansion should not affect the performance of the service and other provisions it should be done to improve the service and its performance. Analysing of the cost of benefit is required before taking such a big step of expanding the clinic. The next thing would be to determine if the expansion is happening in the order of identified goals and objectives, or if it isn’t. For example, in the mission statement of improving quality of healthcare services to the community. Approval should be based upon objectives, but before approval there are certain things to keep in mind and into consideration. For instance, identifying the resources available and to also see if the alignment of plans with strategic goals of the clinic, therefore the annual plan would only be approved when it would satisfy the Board (HSE,2012) John (pseudonym) presented at his G.P recently with feeling of anxiety, emotional flatness, and worthlessness and adnominal pain. John is a 44-year-old male and lives with his Mother his Father died suddenly when he was 4 years old. On assessment G.P diagnosed John with constipation and advised him to take lactulose 10ml b.d but also referred him to the acute psychiatric unit as John had disclosed that he had thoughts of suicidal ideation. John was known to the psychiatric services as he had been an inpatient there on any occasions. Psychiatric Services On the unit in which the author works there are many patients admitted for detox treatment due to alcoholism. John was admitted on numerous times, he was not always an alcoholic but started drinking after his son died by suicide a few years ago. His Mother is at her wits end and each time he is admitted he says he will no longer drink alcohol. On this admission he complained of great discomfort due to constipation which his G.P had diagnosed but he had not been written up on his Kardex on admission for this and it could not be administered. He also voiced that his withdrawal from alcohol symptoms are very bad. The nurse at hand over reveals he is frequently asking for analgesia to ease the discomfort of the constipation and Librium to settle “his nerves”, she reveals him as been agitated and a bit of a nuisance and since the author has cared for the patient before she knows this is out of character for him. The author also knows from previous experience of caring for the patient that he requires more frequent administration of Librium than some detox patients. On starting duty, the author goes and talk with the patient, he appears restless when asked how he is feeling he reveals he really needs to get off the drink and goes on to say his Mother is on her “wits end” and that he really needs to succeed this time in stopping drinking. He also shared that he is in pain but nobody is listening to him, his symptom of the shakes and sweats appear obvious.
