St. Louis, Missouri: Saunders Elsevier Child ... and Family Health Assessment (Chapter 2) Foundations for Child and Family Assessment • The assessment is based on development, family structure and function, culture, anatomy and physiology, pathophysiology, pharmacology, health care delivery systems and functions, communities, and standards of primary health care for children. 1. Domains of health care problems a. Alternate model of assessment to incorporate functional health patterns and developmental issues. b. Three distinct domains: developmental problem, functional health problem and diseases. i. Ex: ii. Developmental problems 1. Long-term issues of development and maturation over the lifespan. a. 15% of children are at moderate risk for developmental, behavorial or social delays b. 11% are at high risk c. Asses for problem in gross motor, fine motoe, speech and language, cognitive, social/emotional, and adaptive behavoirs iii. Functional health problem 1. All functional health problem involve the family, therefore assessment of the caregivers knowledge and experience contributes to the problem 2. Gordons 11 patterns a. health beliefs and behavior b. nutrition c. elimination d. activity e. sleep f. role relationships g. coping h. self-perception i. cognition and perception j. sexuality k. values and beliefs 3. Labels for health problems a. NANDA taxonomy i. Updated every 2 years ii. Includes the ICD-10- CM, CM stands for clinical modification iv. Diseases 1. Conditions assessed and managed at the tissue or organ level a. Example: Otitis media, streptococcal pharyngitis, and appendicitis b. Use ICD 10 i. Is limited in the behavioral, social and developmental problems v. Problem interactions 1. The context of the problem is not always the problem listed. a. Example: diagnosis id Iron deficiency anemia. The problem can be nutrition: less that body requirements for iron, then interventions can be teaching of foods. 2. Developmental Assessment Foundations a. Developmental monitoring (Surveillance) b. Developmental screening and assessment i. Screening 1. Language 2. Motor 3. Social-emotional 4. Cognitive skills 5. Regulatory- infants’ daily patterns of sleep-wake cycles including sleeping, eating, moving, responding, and reacting to internal and external environments. 6. Sensory system ii. Strategies specific to developmental screening 1. Standardized screening done at 9months, 18 months and 24-30 months. a. Screening tools i. Ages & Stages Questionnaires, Third Edition (ASQ-3) ii. Ages & Stages Questionnaires: Social- Emotional (ASQ:SE) iii. Parents' Evaluation of Developmental Status (PEDS) iv. Modified Checklist for Autism in Toddlers (M-CHAT) v. Edinburgh Postnatal Depression Scale (EPDS) vi. Pediatric Symptom Checklist (PSC) vii. Patient Health Questionnaire-9 (PHQ-9) viii. CRAFFT and Patient Health Questionnaire-2 (PHQ-2) are recommended for teens c. Developmental assessment- second-level analysis focusing on a narrower, often complicated problem. Used to confirm a developmental problem, identify the type of problem, describe the level of functioning in one or more developmental domains, and provide guidance and referrals, interventions, or resources. i. Strategies specific to developmental assessment- often require specialized training or referral to specialist. 1. Tools used for overall development i. Bayley Scales of Infant Development ii. The child developmental inventory iii. Mullen scales of early learning 2. Tools used for specific behaviors or characteristics i. Autism diagnostic observation scale-generic (ADOS-G) ii. Childhood autism rating scale (CARS) 3. Family assessment foundations i. The family’s role in health care of children i. Factor such as level of education, beliefs and attitudes about health influence children health status ii. Maternal depression 1. Poor caregiving that can result in: a. Poor language development at 3 years old b. Predictive of asthma especially in inner-city African American families ii. Family Assessment basic elements i. Family structure and roles 1.
