Sickle Cell Anemia Case Study Chamberlain University College of Nursing NR324: Adult Health 1 Sickle Cell Anemia Case Study T.D. is a seventeen-year-old, African American, female who prese... nts to the emergency department with complaints of pain in her thoracic and abdominal regions of a 9/10, and feeling nauseous after taking a rather hard fall during a soccer game. She was diagnosed with sickle cell anemia when she was three years old, and has had no other history of disease. She has been previously admitted to the hospital with exacerbations of Sickle Cell Disease. Upon admission, the nurse examines T.D. and notes she is febrile with edema of the chest and abdomen, is hypertensive and tachypneic with pallor of the mucous membranes. (Harding, et. al., 2019) Laboratory/Diagnostic Testing A blood sample is required for a peripheral blood smear to determine the type and severity of the anemia (Harding, et. al., 2019). The peripheral blood smear showed an increase in reticulocytes; the normal range of reticulocytes is 0.5%-2% (Pagana, et. al., 2018). Complete Blood Count (CBC) tests will also show a decrease in hemoglobin and hematocrit, and an increase in bilirubin (Emory University, 2021). The normal range of hemoglobin for females is 12-16 g/dL, 37-47% for hematocrit, and 0.3-1 mg/dL for bilirubin (Pagana, et. al., 2018). A Doppler study on the extremities can also be done to determine if there is any presence of a deep vein thrombosis (Harding, et. al., 2019). Collaborative Management Collaborative management for T.D. will consist of a physician, a nurse, a patient care assistant and a hematologist due to sickle cell being a disease that relates blood. Our collaborative team responsibilities include pain management, preventive and primary care issues, and patient education.
