The focus of this case is the transition from the... acute care setting to home, and understanding that older adults are vulnerable during such transitions is fundamental to ensuring competent, individualized care for older adults and their caregivers. Millie has several chronic conditions, in addition to the urinary tract infection that she arrived with, and she also exhibits the geriatric syndrome of incontinence. In addition, information from assessments done the day before indicates that Millie will need help for some activities of daily living. The impact of these conditions, geriatric syndromes, and physical limitations should be considered as the transition to home is planned and coordinated with Millie and Dina. Millie is going home with a home health referral, and verification that the referral is completed is a component of case management and coordination.
