Case Study Part 2 Past Medical History: The patient’s blood type is AB positive. Medical: The patient has a history of high blood pressure. The patient is also diabetic. They were diagnosed w... ith both conditions are the age of 30. Social: The patient usually travels around for work so they do not really get to cook for themselves and often finds themselves eating outside or ordering takeout. The patient does not smoke. Family: The patient notes that both her parents have problems with high blood pressure and that their mother is also a diabetic. There is also a history of heart disease that runs in the family. Surgery: The patient has no previous record of surgery Medications: The patient uses metformin to manage their diabetes. Vaccines/Allergies: The patient has a pollen allergy, and they are up to date with their vaccines and recently got a flu shot. Review of Systems: Chest/Respiratory: 1. Do you have any trouble catching your breath or feel short of breath? I do not usually feel short of breath, but after exercising it takes me a bit longer to catch my breath now. 2. Do you ever have pain when you take a deep breath? I do not feel pain when I take a deep breath. This is a class assignment and is for educational purposes only. Do you use antacids? Do you use laxatives? I occasionally take antacids for heartburn, but I do not take laxatives. 3. How about bowel habits? Have they changed recently or have noticed blood or black colored stools? I have been using the bathroom a bit less than usual and have been feeling slightly constipated, but the color is normal.
