Case Study by Maybelle Holt, BSN, RN NR 601- Chamberlain University Mrs. Wong is a 59-year-old Asian female coming in the clinic for her three-month follow- up of for her right knee arthritis. She... states having less knee pain with increase in mobility but has concerns of increasing fatigue for the past 2 months and a 4lbs weight gain despite light lifting and walking on a treadmill for 30 minutes. The goal of this paper is to evaluate objective and subjective findings to be able to diagnose and create a plan of management by applying evidence-based guideline. Contents of this paper will include diagnostics, medications, education, referrals, and costs of medications including prescribed and over-the-counter (OTC). Assessment Mrs. Wong is a 59-year old Asian female here for follow-up of right knee arthritis with new complaints of fatigue, 4lbs weight gain, increased hunger and thirst. A height of 5’1.5” and weight of 165 pounds puts her with a BMI of 30.7, which indicates that she is obese (CDC, 2019). Her fasting morning labs showed the white blood count, hemoglobin and hematocrit, liver and kidney function, and thyroid levels within normal limits. Her fasting plasma glucose, hemoglobin A1C, and lipid panel all revealed abnormal levels. The urinalysis result showed 1+ glucose. Primary Diagnosis – Type 2 Diabetes Mellitus Without Complications (E11.9) pathophysiology Type 2 diabetes is correlated with insufficiency of secretions of insulin. This is due to b-cell dysfunction from inflammation and metabolic stress linked with genetic influences. Indications of Type 2 Diabetes are polyuria, polydipsia, polyphagia, weight gain and fatigue (Pippitt et al., 2016). [Show More]
