The last seven weeks has given me the opportunity to improve and continu... e to build knowledge and understanding of the importance of creating a treatment plan based on evidence based guidelines that can measure outcomes of current and future patients. This not only addresses the patient needs but it also protects and promotes the role of the advanced practice nurse as a profession. Healthcare is an ever-changing practice and it is essential that nurse practitioners can practice at the highest level of care. MSN Essential IV: Translating and Integrating Scholarship into Practice Recognize that the master’s-prepared nurse applies research outcomes within the practice setting, resolves practice problems, works as a change agent, and disseminates results. My clinical experience in retail health was interesting and allowed me to see a different side of healthcare that I’ve yet had a chance to experience. I was not aware of the limitations that are placed on providers in this setting. As the main provider you must be able to take on more than one role while still utilizing evidence based guidelines. This allowed me to be a very important part of the team by, triaging the patients, collaborating with my preceptor to develop the plan of care, and education the patient regarding their condition and how the treatment plan would benefit them. This course has reinforced the nurse practitioner’s responsibility to coordinate and maintain patient-centered care while collaborating with other healthcare providers within the organization. I feel confident and empowered to create an environment of trust, open communication, knowledge sharing, problem solving, and decision making in collaborating across multiple levels of healthcare. (Sangster- Gormley, Griffith, Schreiber, Borycki, Feddema, & Thompson, 2015). I have learned the importance of developing and implementing policies, and making changes as needed. Advance practice nurses are responsible for advancing awareness of the profession and the value it brings to healthcare. Although my preceptor was more knowledgeable she had difficulties with skin conditions. I felt a little more comfortable with skin we would coordinate care for the patient and work together to develop a treatment plan based on evidence; this allowed me the opportunity to share some of the things I’ve learned by serving as a role model and a mentor for her when dealing with skin rashes meeting the MSN Essential IV competency (AACN, 2011). I was also able to follow up with patients and monitor outcomes via 48 hour callbacks that we performed on every patient seen in the clinic. If patients had not improved, we would then go back and review their case. During this review, my preceptor allowed me to ask questions and use my critical thinking skills to develop differential diagnosis’s that we may have missed. This process helped me to develop a stronger evidence based foundation meeting the MSN Essential IV of scholarship (AACN, 2011). Nurse Practitioner Core Competencies # 7 Health Delivery System Competencies 1. Applies knowledge of organizational practices and complex systems to improve health care delivery. 2. Effects health care change using broad based skills including negotiating, consensus-building, and partnering. 3. Minimizes risk to patients and providers at the individual and systems level. 4. Facilitates the development of health care systems that address the needs of culturally diverse populations, providers, and other stakeholders. 5. Evaluates the impact of health care delivery on patients, providers, other stakeholders, and the environment. 6. Analyzes organizational structure, functions and resources to improve the delivery of care. 7. Collaborates in planning for transitions across the continuum of care. I met the health delivery system competencies throughout my rotation in retail health. The site where I was had limited lab testing available on site and was not equipped to perform EKG’s. Patients requiring further evaluation were referred to their PCP or the emergency room depending on severity of their condition. This helped to “minimize risk to patients and providers at the individual and systems level” (The National Organization of Nurse Practitioner Faculties [NONPF], 2017). For example, I saw an older Hispanic male that came in for complaints of blood in his urine on and off for about 3 weeks. This patient had not seen a PCP in many years as he states he never gets sick. During his exam, we obtained a urine sample which was frank blood. This patient needed to be evaluated at a higher level of care than we could not provide in the clinic. This patient was ultimately referred to the nearest ER. I was able to call report to the receiving provider, answer any questions he had, and get the patient evaluated urgently. This exemplified the process of collaborating and planning for transitions across the healthcare continuum (NONPF, 2017). During my clinical rotations, I was able to present my assessment findings, a proposed treatment plan, and develop my clinical judgement skills with the guidance of my preceptor. She allowed me the latitude to exercise clinical decision making skills, critical thinking, and develop a treatment plan that was based on national guidelines. This allowed me to become more comfortable with utilizing guidelines to treat common conditions encountered daily in the primary care setting. During clinical hours, my preceptor and I performed multiple sports physicals and immunizations for children and college aged students returning to school. I initially was somewhat confused as to why we were performing these physicals when they had PCP’s where their insurance would cover the cost vs paying out of pocket. The main reason was that it was so hard for parents to get appointments with their PCP; some patients appointments were 3-4 months out, so it was easier to use the retail clinic. In analyzing the organizational structure, function, and appropriate use of resources this is a process that can use improvement, not only for the patient but for the system. Patients need to be seen in a reasonable amount of time. As a master’s prepared nurse, it is my duty and responsibility to strive for improvement in healthcare cost, delivery and outcomes. The case studies and clinical settings created a learning environment that demonstrates the value of teamwork, implementing evidence based theory and research, and learning to work out challenges in the work place. I have also learned how organizational structure and principles of management including outcomes affect the way an organization views their practice. These last seven weeks have created a learning process of patient care and the challenges practitioners will face from time to time. However, it also provided a strong knowledge in research, application of research, facilitating change, and consistently working to improve the delivery of care by collaborating in the planning and implementing of care. Dr. Stucki, I have really enjoyed this class! Your interactive style of teaching and pushing me to dig deeper for answers made this one of my favorite classes.
