In addition to the day in, day out routine that should involve the patient and their family, PCC can also impact business practices, regulatory requirements, and reimbursement. One way business practices are impacted through patient centered care (PCC) increased efficiency. A healthcare institution that is operates based on the input and needs of the patient and their families will be more efficient. PCC inherently should involve all stakeholders, which can reduce waste of resources and staff by decreasing process delays, maximizing patient education efforts, and decreasing diagnostic tests. According to the Becker Hospital Review (2015), the Gallup Management Online Journal showed that patient and family involvement which originated through a PCC approach consistently improved hospital performance and business outcomes, which impacted long term earnings. Another way business practices are impacted is through recruitment and retention of staff. Hospitals implementing a PCC approach improve employee satisfaction and retention. This is mostly due to an efficient system, which decreases employee burnout and improves relationships (Kimball, 2015). Patient centered care can also have an impact on regulatory and accreditation requirements. Hospital surveys are able to measure components of a healthcare institutions PCC program to determine how and if the standards are being met, which can have an impact on survey results, and even accreditation decisions. Patients are provided multiple opportunities to respond to patient satisfaction surveys, from an inpatient stay, their experience with the service on the telephone, satisfaction with the food services, to their satisfaction with parking. Information from these surveys is collected, aggregated, and trended. Feedback is then sent back to the hospital in the form of quality indicator reports. Regulatory and accrediting bodies then review this data as part of the quality and performance improvement review process (AHRQ). Implementation of PCC can also contribute to compliance with Joint Commission standards. The Joint Commission has multiple standards and elements of performance which directly relate to PCC, for example: Provision of Care chapter, Element of Performance 02.01.21, The hospital effectively communicates with patients when providing care, treatment and services, and Rights of the Individual chapter, Element of Performance 01.02.01, The hospital respects the patient’s right to participate in decisions about his or her care, treatment, and services (AHRQ,). Implementation of PCC also has a direct impact on financial reimbursement. As mentioned previously, accrediting and regulatory bodies are surveying implementation of patient centered care. According to the Becker Hospital Review, Centers for Medicare and Medicaid Services (CMS) plans to link hospital performance directly to reimbursement in the future. Additionally, the government mandates all hospitals to evaluate quality and patient satisfaction, then apply reimbursement penalties if the facility fails to meet standards (2015). B1. A small, rural healthcare setting was utilized for completion of the Patient and Family Centered Care Assessment Tool. This facility has an Urgent Care, small 13 bed inpatient unit, 38 bed long-term care unit, Behavioral Health and Mental Health services, as well as many Primary Care clinics. Ancillary services are available, such as laboratory and radiology. The population served consists of mostly Caucasian and African American male patients, with a growing female patient population. Nearly all of these patients have multiple diagnoses, both physical and mental health, and multiple comorbidities. There is a strong need for management of co-chronic diseases (i.e. congestive heart failure, chronic obstructive pulmonary disease, diabetes mellitus), as well as post-traumatic stress disorder. The current PCC program is in its infancy, and is essentially nonexistent. The inpatient unit has implemented bedside reporting, which includes discussing the plan of care for that particular day, and the patient’s hospital stay up to that point. The patient is given the opportunity for the family to be present, which includes them in their healthcare decision making. While patient and family participation is growing in the everyday healthcare decisions, there is little to no participation in the committees, councils, and boards which make decisions for the healthcare institution .....................................................................................continued.........................................................................
