Professional nurses rely on research findings to inform their practice decisions; they use critical thinking to apply research directly to specific patient care situations. The research process allows nurses to ask and answer questions systematically that will ensure that decisions are based on sound science and rigorous inquiry. Nursing research helps nurses in a variety of settings answer questions about patient care, education, and administration. As you contemplate your role in the research process, read the following article. 1. Choose one case study, and formulate one searchable, clinical question in the PICO(T) format. There are several potential questions that could be asked. 2. Identify whether the focus of your question is assessment, etiology, treatment, or prognosis. 3. Remember to integrate references. When developing a clinical question in the nursing field to acquire adequate evidence-based knowledge to implicate, the necessary considerations to developing this question, as discussed in the Chamberlain Week 1 lesson are the “…who, what, where, when, why, and how of the situation…” (Chamberlain, 2018). The ideal tool to be used would be the PICO(T) template, and this template, as discussed in the lesson breaks down to the population, intervention, comparison, outcome, and occasionally time of an interest. (Chamberlain, 2018). Nursing today requires a substantial amount of evidence-based practice which is “The use of the best scientific evidence, integrated with clinical experience and incorporating patient values and preferences in the practice of professional nursing care” (Houser, 2018, pg. 13). Without implementation of evidence-based practices and procedures in the hospital setting, the ideal of an optimal quality of care for our patients would not be achievable since their safety could be improved through this research. The case study that I chose to formulate a research question for was in the case of the 80-year-old retired army officer. This patient described in the Type 1 diabetes mellitus-common cases article by S. Kumar lived alone and was diagnosed with type 2 DM for the last 12 years with no aid to control from oral diabetics and insulin management. This patient had an attendant who would come and administer his injections for him but was not regularly scheduled which caused the patient to frequent hypoglycemia. Population of interest – Elderly population 70+ diagnosed with Type 2 diabetes mellitus Intervention – Education and utilization of methods towards controlling and monitoring glucose levels for stabilization of HBA1C levels by introducing degludec and dietary modification. These interventions introduced for an optimal goal of preventing hypoglycemic events in the patients. Comparison – original diabetic control methods (oral diabetics and insulin) versus the transferring to degludec management Outcomes – after a period of time, full management of patient’s glycemic index under singular diabetic medication degludec. Time – followed patients for at least 3 months and monitored HBA1C for steady levels during this time period. Then followed regularly. With the elderly, certain aspects of diabetes monitoring can become difficult. The CDC provides a pamphlet on tips to help these patients stay healthy with a section called Know what to do every day:  Take your medicines even when you feel good. Tell your doctor if you cannot afford your medicine or if you experience any side effects.  Check your feet every day for cuts, blisters, red spots, and swelling. Call your health care team right away about any sores.  Brush your teeth and floss every day to keep your mouth, teeth, and gums healthy.  Keep track of your blood sugar and keep a record of your numbers if your health care provider says this is appropriate. Check with your health care team on how often and when to check your blood sugar.  Check your blood pressure if your doctor advises and keep a record of it.  Don’t smoke. Ask for help to quit.
