P- Adult Med/Surg/Tele patients I-Total Patient Care Nursing C-Team Nursing O-Safety and Quality of care T- 3 months Professor and Class, I currently work in a 24-hour observation 12 bed unit. Our patients are adult Med/Surg/Tele patients who may not meet the criteria to be admitted but need further workup to decide future care. On a good day these patients are alert, oriented and ambulatory. Since the Affordable Care Act our membership has grown but our facilities have not. Many days now we have admitted patients holding in our unit waiting for beds to become available in our in-patient units. Since the beds are first come first served, in regard to orders, our unit sees a much different population. Majority of days we will have 4 or more bed bound patients. In the in-patient units they are designed to function using the team nursing model. The in-patient unit has a unit clerk, certified nursing assistants(CNA), transporting nurses/techs, resource nurse and break nurses. The observation unit is designed using the total patient care nursing model. As nurses we are responsible for all aspects of care for each patient. This includes clerk work, CNA work, transporting, breaks and primary nursing tasks. We are lucky that we are in California and have a 4:1 patient to nurse ratio. We are also fortunate to be staffed with one extra nurse for break/transports. On days with multiple total care patients though this is not enough to meet the needs of all the patients. This last year we have had multiple complaints on delay of care and have experienced multiple falls. The team nursing model is defined as nursing delivered by a team of nursing professionals with a mixed set of skills.(Hughes, 2018). While total care nursing is patient-centered and depends on on the nurse to deliver all the care needed. (Hughes, 2018). When I used my PICOT question to search for evidence I found an article titled “Associations of patient safety outcomes with models of nursing care organization at unit level in hospitals”. This article is relevant to my clinical issue because it researches each nursing care model and the impact on patient safety.
