ATI PN VATI PHARMACOLOGY FOCUS REVIEW  Do not administer live virus vaccines (varicella or MMR), to a client who is severely immunocompromised. Severe febrile illness is a contraindication to all ... immunizations. Moderate or severe illnesses with or without fever are also precautions to receiving immunizations.  Chronic neurological disorders include Parkinson’s disease (PD) and seizure disorders. Medications for treatment help manage manifestations and improve quality of life. Levodopa/carbidopa is the most effective treatment for (PD), but beneficial effects diminish by the end of year 5.  Isoniazid is highly specific for mycobacteria by inhibiting growth of mycobacteria and preventing synthesis of mycolic acid in the cell wall. Isoniazid is indicated for active and latent tuberculosis. Clients who have liver disease should not take isoniazid. Use cautiously in older adults and clients who have diabetes mellitus or alcohol use disorder.  Heparin treats disorders necessitating prompt anticoagulant activity (evolving stroke, pulmonary embolism, massive deep-vein thrombosis). Complications include hemorrhage due to heparin overdose. Monitor vitals signs and withhold heparin if client needs protamine IV (antidote), and should avoid other anticoagulants, including aspirin. Check aPTT every 4 to 6 hours during initial therapy and once daily thereafter. Therapeutic response raises the aPTT by 1.5 to 2 times (60 to 80 seconds) 1.
