In medical terms, care for acute health conditions is the opposite from chronic care, or longer term care. Subacute care serves patients needing complex care or rehabilitation. Subacute care is defined as comprehensive inpatient care designed for someone who has an acute illness, injury or exacerbation of a disease process. Hospice care provides medical services, emotional support, and spiritual resources for people who are in the last stages of a serious illness, such as cancer or heart failure. Hospice care also helps family members manage the practical details and emotional challenges of caring for a dying loved one. • Review factors that institute Lifestyle changes and health promotion To achieve lifestyle change that promote health, a person need to be willing to implement change.
