She has had a 3 day history of nasal drainage, cough, with poor PO intake. Lily is a 6- month-old Caucasian female born at 38 weeks. Learning Objectives 1. Identify the primary nursing diagnosis. 2. Recognize indicators of a respiratory condition. 3. Anticipate physician/nurse practitioner orders for this client. 4. Communicate changes of status to health care team using SBAR tool. 5. Implement nursing interventions for clients with a respiratory condition. 6. Communicate effectively with client and family. 7. Prioritize immediate nursing interventions based on accurate assessment and analysis of data. Pre-Simulation Ticket You are about to participate in a simulated patient case. The child has a chief complaint - We expect you to assess the patient and manage her to the best of your ability. Treat the simulated child and her family as actual humans. This case lasts approximately 20 minutes. We will debrief as a team following the case. 1. List the child's chief complaint and history of present illness? 2. What data is relevant to this child and needs to be addressed immediately? What is the rationale for addressing these needs immediately? (It is okay to list here, but be sure to include the rationale.) 3. What is the most likely medical problem(s) that is occurring with this child? Please describe what you are thinking. 4. Explain the pathophysiology of a respiratory condition. 5. Write two nursing diagnoses with the indicating factors. 6. What interventions will you initiate for this child and family? 7. What physician orders do you anticipate? Provide not only the order you anticipate, but the rationale for the order(s). You need to include a minimum of ten orders with rationales. (It is okay to list here, but be sure to include the rationale.) 8. List the normal lab values/diagnostic tests which are commonly obtained for a respiratory condition. Think about items that may help aid in making a diagnosis. 9. When performing your initial assessment and obtaining the initial set of vital signs, what do you expect to find? (You need to know how to perform an assessment and how to obtain vital signs on this child!) 10. What safety equipment should you ensure is present and working at the bedside? 11. What do you expect to see in this child, in regards to growth and development? 12. How will you integrate the family's preferences or values into the plan of care?
