Taking the pulse for a full minute provides the most accurate HR. (1) Six-second strip method: Count the number of QRS complexes (12) and multiply by 10; therefore, 10 x 12 = 120 bpm. (2) Large block method: You can approximate the HR by choosing any two consecutive QRS complexes and counting the number of large boxes (almost 2.5 in this strip). 300 (number of large boxes in a minute) divided by 2.5 large blocks = HR; therefore, 300 2.5 120 bpm. (3) Small block method: There are 12.5 small boxes between QRSs. 1500 (number of small boxes in 60 seconds) divided by number of small boxes between QRSs; therefore, 1500 12.5 = 120 bpm.
