Diagnosed with DM at 24years old, noncompliant with metformin and checking blood sugar regularly, poor diet control Diagnosed with Asthma at 2.5years old, taking PRN inhaler for exacerbations, currently - PRN Inhaler is ineffective Heavy-flow periods with cramping, and breast tenderness. Pt has irregular periods (6 periods/year), lasting 9-10 days Immunizations current, received childhood vaccinations Flu shot received 5-6 years ago Last tetanus vaccine a year ago Pap-smear completed 4 years ago, normal No current eye exam, last time for an eye exam was when she was little Has not seen dentist in a few years, went twice a year as a child Social History (Soc Hx) - Includes but not limited to occupation and major hobbies, family status, tobacco and alcohol use, and any other pertinent data. Include health promotion such as use seat belts all the time or working smoke detectors in the house. Denies smoking and current recreational drug use, admits to smoking pot when 20-21 years old, pt stopped because it disturbed her asthma Pt is exposed to secondhand smoke occasionally Pt drinks occasionally, about 6 drinks per month. Currently working on bachelors degree for accounting Lives with mom and sister, family is a good support system Began sexual activity at age 18, has had 3 sexual partners (males), “sometimes” uses a condom Family History (Fam Hx) - Includes but not limited to illnesses with possible genetic predisposition, contagious or chronic illnesses. Reason for death of any deceased first degree relatives should be included. Include parents, grandparents, siblings, and children. Include grandchildren if pertinent.
