Hospital Discharge Plan Title Student's Note Write what you would say to instruct Tina about managing her diabetes. Ms Jones, before we can discharge you, I would like to provide some important information regarding managing diabetes with you. It is important that you regular monitor your blood glucose at home and keep up with your medication schedule. Always feel free to reach out to our doctor's office if you have any concerns regarding your medications. Monitor your blood sugar at home at specific times, d a test before each meal and 1 or 2 hours after a meal. Before, during and after physical activity. Detecting signs and symptoms of hypo- and hyperglycemia are crucial as well. These symptoms can occur rapidly, thus detecting them early is favorable. Hypoglycemia is characterized by hunger, trembling, nausea, sweating and a fast heartbeat. When you notice these signs of low blood sugar, drinking fru juice or anything with a good amount of carbohydrates is recommended. Signs an symptoms of hyperglycemia include fatigue, increased thirst and urination. By managing your diabetes with the prescribed medications, we can prevent commo complications of diabetes. When caring for your feet make sure to check for lesions and be careful with clipping your toenails, preferab get them done by a podiatrist to prevent any bleeding complications. Besides medication, I would like to stress the importance of regular exercise and diet. Lim foods that are high in sugar, salt and fat, make sure to track the amount of carbohydrates as well. Try smaller portions of food spread throughout the day. Create a meal plan with a variety of healthy foods, containing vegetables, fruits, grains, protein and non- or low fat dairy products. Educate Tina on the proper technique and procedure to be used to clean and maintain her wound. Ms. Jones, let's talk about wound care. You will need to clean and change dressin on your foot wound twice a day, once in the morning and once before bedtime. To ensure healing, the wound dressing change should be done in a clean environme Make sure to gather all the supplies and wash your hands thoroughly with antibacterial soap for 1-2 minutes. When you remove the dressing, examine the wound and take note of any changes, color, smell, consistency of drainage if applicable. Dispose of the old dressing. Wash your hands again and put on a pair sterile gloves. Dip a few 4 by 4 gauze pads in the betadine-water solution and use these to wipe the wound clean. Dispose of them with each wipe, do not go back a forth (wiping). Pat the wound dry with a dry piece of gauze. Place a clean and dry by 4 gauze and wrap the bandage dressing around the ankle. Ms. Jones, we will now discuss the prescribed medications that you will take hom For your asthma, an Albuterol inhaler is prescribed, which you have to take 2 puff every four hours and as needed when you experience any coughing, wheezing a shortness of breath. The albuterol will help open your lungs to maintain a clear airway, making breathing easier. Usage of the inhaler will improve your symptoms especially when you're around known triggers, such as cats, as you mentioned before. You may notice side effects such as increased blood pressure an heart ra when using this medication. For your diabetes management, Insulin glargine and Metformin are prescribed. Insulin glargine is a long-acting insulin which will work thoughout the day. It is important to check your blood sugar before administering any insulin. If your blood sugar is lower than 100mg/dL, you may want to withhold the insulin for a few hours to prevent hypoglycemia and reassess your blood suga The insulin glargine is 10 units taken at bedtime. Similarly for metformin, you hav check your blood sugar prior to taking it. The prescribed dose for your metformin 850mg by mouth twice a day. You may notice gassiness and loose stools when y start taking it, but it should improve over time. For treating your infection, Linezoli 600 mg by mouth every 12 hours for 10 days is prescribed. This is an antibiotic which you should finish taking, even if you believe the infection is cleared, to prev any reoccurence. You may experience an increase in heart rate, hypersensitivity, headaches, dizziness or confusion when taking this. For pain management, ibuprofen 800mg tablet by mouth three times a day as needed is prescribed. Ofte with pain medication, you may experience constipation. Make sure to drink adequ amounts of water to counteract the possible constipation. If the pain is not manag with the prescribed dosage, please contact the primary care provider. Tell Tina about the kinds of accommodations she will need to make to her routine and to her environment. Remind Tina of her risk of falling due to medications and impaired mobility. Discuss moving items that would put Tina at increased risk of falls such as area rugs and clutter. Ms. Jones, the doctor has given the order to let you resume your normal activities tolerated. You may also bathe. Please ask for assistance if needed. With the injur on your foot, it may be difficult to move around, move any potential obstacles in th house that could be a hazard. Any rugs or clutter are advised to be moved away temporarily. If you have nightlights at home, it may be a good idea to put them to use, especially near the hallway towards the bathroom. Keep in mind that side effects of the prescribed medication can cause drowsiness or confusion which lea to an increased risk for falls. The diet that has been ordered for you is called the ADA 1800 calorie diet. ADA stands for American Diabetes Association. This diet is specifically created for diabetics in mind. You are advised to stay within 1800 calories a day, limiting saturated and trans fats, sugars and salt intake.
