History taking begins before physical examination and usually continues throughout the examination. Diagnostics involve tests and screens to confirm or determine the presence of a disease (Kennedy-Malone et al., 2019). Nutritional assessment entails four components: 1) nutritional history using a validated tool; 2) food intake diary of 1 to 3 days; 3) physical assessment, including anthropometric measurements and signs of nutritional deficiencies; and 4) biochemical markers (Kennedy-Malone at al., 2019, p. 28). The last component is medication review, which should be completed at every visit, and the most frequently used criteria for evaluating drug use is the Beers Criteria for Potentially Inappropriate Medication Use in Older Adults. The Mini Nutritional Assessment is a screening tool is used to identify older adults, greater than 65 years old, who are malnourished or at risk for malnutrition. (Young, Mudge, Banks, Rogers, Demedio, & Isenring, 2018). There are six questions that consist of topics on food intake, weight loss, mobility, psychological stress or acute disease, presence of dementia or depression, and body mass index. Scores of 12-14 are considered normal nutritional status, 8-11 indicate at risk of malnutrition, and 0-7 indicate malnutrition (Young et al., 2018).
