According to the American Sentinel University (2011), there are three types of nursing sensitive indicators that are structure, process, and outcomes. Structural indicators are comprised of nursing staffing, the staff skill level, education levels and experience of nursing staff, as well as staffing mix. Process indicators pertain to methods of patient assessment and nursing interventions, as well as nursing staff satisfaction. And finally outcome indicators reflect patient outcomes directly related to the quantity and quality of nursing care (para. 2). In this scenario, nursing staff seems to not have well understood the nursing – quality indicators, the prevalence of restraints and pressure ulcer could have been avoided leading to a patient satisfaction outcome. In order to prevent pressure ulcers, a Braden scale could have been used to assess Mr. Js risk for skin breakdown/ pressure ulcer then a complete skin assessment could have been conducted using two registered nurses as witnesses every shift as we do on my unit to identify possible signs of pressure ulcer development and take proper measures to avoid further injury.
