Celecoxib: Celebrex, Consensi, Elyxyb, Seglentis: MECHANISM OF ACTION, CONTRAINDICATIONS, SIDE EFFECTS, ADVERSE EFFECTS, and NURSING RESPONSIBILITIES Unlike most NSAIDs, which inhibit both types of ... cyclooxygenases (COX-1 and COX-2), celecoxib is a selective noncompetitive inhibitor of cyclooxygenase-2 (COX- 2) enzyme. COX-2 is expressed heavily in inflamed tissues where it is induced by inflammatory mediators.The inhibition of this enzyme reduces the synthesis of metabolites that include prostaglandin E2 (PGE2), prostacyclin (PGI2), thromboxane (TXA2), prostaglandin D2 (PGD2), and prostaglandin F2 (PGF2). Resultant inhibition of these mediators leads to the alleviation of pain and inflammation.By inhibiting prostaglandin synthesis, non-steroidal anti- 1. Celecoxib is contraindicated in patients with salicylate hypersensitivity or NSAID hypersensitivity who have experienced asthma, urticaria, or other allergic reactions after taking aspirin or other NSAIDs. 2. Celecoxib is contraindicated in patients with known hypersensitivity to the drug, its components, or sulfonamide hypersensitivity (celecoxib contains a sulfonamide side chain). However, celecoxib does not contain the aromatic amine or the N1-substituent that are present in sulfonamide antibiotics.  headache,  abdominal pain,  indigestion,  diarrhea,  nausea,  upset stomach,  bloating,  gas,  dizziness,  nervousness,  headache,  runny or stuffy nose,  sore throat,  skin rash, and  insomnia. Celecoxib, like other NSAIDs may cause serious stomach and intestinal ulcers that may occur at any time during treatment. Celecoxib does not interfere with the function of the blood platelets and, as a result, does not reduce clotting and lead to increased bleeding time like other NSAIDs. Allergic reactions can occur with celecoxib.  Instruct patient to take celecoxib as directed. Do not take more than prescribed dose. Increasing doses does not appear to increase effectiveness. Advise patient to read Medication Guide before starting therapy and with each Rx refill in case of changes.  Caution patient to avoid use of more than one NSAID or aspirin at a time; increases risk of GI toxicity. In addition, celecoxib has been found to inhibit carbonic anhydrase enzymes 2 and 3, further enhancing its anticancer effects. As mentioned in the pharmacodynamics section of this drug entry, celecoxib may cause an increased risk of thrombotic events. The risk of thrombosis resulting from COX-2 inhibition is caused by the vasoconstricting actions of thromboxane A2, leading to enhanced platelet aggregation, which is uncontrolled when the actions of Individuals who have developed allergic reactions (rash, itching, difficulty breathing) from sulfonamides (for example, sulfamethoxazol e and trimethoprim [Bactrim]), aspirin or other NSAIDs may experience an allergic reaction to celecoxib and should not take celecoxib. (abdominal pain, black stools), skin rash, unexplained weight gain, or edema occurs. Patients should discontinue celecoxib and notify health care professional if signs and symptoms of hepatotoxicity (nausea, fatigue, lethargy, pruritus, jaundice, upper right quadrant tenderness, flu-like symptoms) occur.  May cause hypertension. Instruct patient in correct technique for monitoring BP and to notify health care professional if significant changes occur.  Inform patient of increased risk of MI and stroke. Use lowest effective dose for shortest time. Advise patient to notify health care professional immediately if signs and symptoms (shortness of breath or trouble breathing, chest pain, weakness in one part or side of body, slurred speech, swelling of the face or throat) occur.  Rep: May cause fetal harm.
