Is the discrepancy associated with the red cell testing or the serum testing? State the reasons for this selection. The discrepancy is associated with the serum testing because it is shown an extra reaction between B-red cells and antibodies of the serum. The forward testing indicated that the patient has B group, therefore there should not be B antibodies in the serum that react with the B-red cells aggregated in the reverse testing. 2. How would you classify the category of ABO discrepancy shown in this problem? The category of ABO discrepancy shown in this problem can be classified as discrepancies that affect the ABO serum testing. 3. What are the potential causes of an ABO discrepancy in this category? The potential causes of an ABO discrepancy in this category can include: excess of antibodies and missing or weak antibodies. Additional Testing No technical errors were found. The donor’s red cells were washed, and the ABO phenotyping was repeated. Red cell testing results were identical to the first set. In addition to A1 and B reagent red cells, commercial screening cells and an autologous control were tested with the donor’s serum. The results of the testing are depicted in the following table. 4. What conclusions can be drawn from the results of additional serum testing? The additional serum testing confirm that the ABO discrepancy is present. 5. What additional steps are required to resolve this ABO discrepancy? The additional steps that are required to resolve this ABO discrepancy are gather information about the patient’ medical history (diagnosis, historical blood type, transfusion history, and current medication) and look at the ABO typing reaction (weak, missing or extra). Once it is decided if the problem is with red cell testing or with the serum testing, then an investigation is required.
