Allegheny County has an increased percentage of individuals diagnosed with heart failure above the age of 65. Pennsylvania has an average of 13.5 percent of the population diagnosed with heart failure, while Allegheny County has an average of 13.7 percent (Allegheny County, 2020). While this is close to the average, Allegheny County could still continue to decrease the incidence of heart failure. Heart failure costs the nation an average of 34.4 billion dollars annually and is the most common reason for hospitalization in individuals older than 65 (Allegheny County, 2020). Even a small improvement in heart failure management would save the health care system money and improve quality of life for the individuals affected by heart failure. The current heart failure initiative at the Jefferson Hospital, a part of the Allegheny Health Network, aims to decrease 30-day readmission rates. Teaching begins with initial diagnosis and occurs on any subsequent admission that relates to heart failure. Heart failure education focuses on the disease process, preventing exacerbations, lifestyle modifications, medications, follow-up, and diet.
