The body depends on the heart to pump oxygen and nutrient rich blood to the cells. When the heart is unable to pump effectively, it is unable to deliver enough blood to the cells, possibly causing fatigue and shortness of breath, for example. If heart failure gets severe enough, it can make daily activities very difficult. According to the Centers of Disease and Prevention (CDC, 2020), approximately 6.2 million Americans have chronic CHF, with an estimated annual cost of $40 billion dollars for health care (Fleg, 2016). This essay will discuss a final coordination care plan for patients with CHF, including, patient-centered health interventions, ethical decisions in designing health interventions, health policy implications regarding coordination of care, and priorities to establish regarding the plan.
