This report was done more than 72 hours later. In order to provide the best quality and patient safety, it is imperative to inform the condition of the patient when admitted to the hospital. CORRECTIVE ACTION PLAN Joint Commission Standard: PC.01.02.03 Hospital assesses and reassesses the patients™ condition based on their designated time frames. EPs Action Plan Patient first initial hospital assessment must be written with its time frame Medical MD and Nursing Director will assure the policy are reviewed, followed, acknowledged and state the timeframe for completing the initial patient assessment. The policy will be acknowledged and reviewed by all hospital nursing staff. Timeframe: 1 week All patients will be reassessed as necessary should their plan of care or condition change. Note: Reassessment may include a change of diagnosis, treatment, discharge planning, and or desire of care. Nursing Director and Charge Nurse will review the policy and make any updated should the policy state the need for reassessment.
