Patient is married with kids and works as a aeronautic engineer. Patient did not have any new concerns to discuss. Patient is calm, appropriate and cooperative, appears stated age, adequate nutritional status, clear speech and well-groomed and proper hygiene, no obvious deformities observed and no use of assistive devices. Patient denies pain or discomfort, no verbal or visual signs of distress noted. Current prescribed medications include Metformin 500mg Qday and no over the counter medications. Patient states yearly labs are done to monitor A1C levels and current levels are 5.9. Unremarkable childhood illnesses but positive family history of maternal diabetes and paternal hypertension and heart failure. Patient current on vaccinations and has received flu vaccine this season. Patient does have yearly eye and dental exams. On physical examination, vital signs stable and appropriate for age, appropriate height, weight over recommended range, with BMI at 33.9. Skin is warm, dry and intact, no lesions or hyperpigmentation noted. Neurologically, cranial nerves I-XII is intact, and no neurological deficit noted. HEENT reveals no deficits, hearing and vision intact, no abnormalities or deformities noted.
