They organized the care and shared information with other members of the interdisciplinary team. Certain factors can affect the coordination of care such as cultural, physiological, ethical aspect etc. Therefore, each patient should have a care of plan that is specific to them; this is known as a holistic, autonomous, or patient- centered care. Strategies for Collaborating with Patients and Their Families Involving patients and their families in their plan of care facilitate better outcomes for their recovery. According to Goodridge et al (2018), The patients and their family are needed to take active role in their care. Healthcare members can educate them about the management of their illness, but if they do not take the steps in promoting their wellness then the recovery time would take longer. Also, having the point of view of the patient of how they usually handle an illness can also be beneficial to their plan of care. For quality care, nurses also need to make the patient comfortable and respect their cultural differences; it will allow for the patient to accept the care and communicate efficiently with the nurses and physician.
