HEALTH HISTORY2Demographic Data CP is a 26-year-old black male with Haitian-American and Salvadorian decent who grewup in Margate Florida and currently lives with his girlfriend in an apartment that t... hey share. He works 40 hours a week at a furniture company for about 2 ½ years whom he has health and dental insurance with, he states “my job is very demanding and I often get injured and have to dolight duty for the week, which affects my normal paycheck.” CP is also a student at Broward College and is working on getting is Associates in Arts.Perception of Health To Him health means eating healthy, exercising a few times a week, being able to carry out physical activity and being mentally healthy. He believes mental health is sometimes over look and being able to communicate with people and think soundly and not think irrational things such as suicide and hurting others is being in good mental health. Being unhealthy for himis living a sedentary lifestyle, eating unhealthy and being over or under weight, not mentally competent and always being sick.Past Medical History Patient has a history of chicken pox when he was a child patient states he received all his immunization as a child, he has no known allergies. He also had appendicitis in which he had to have an emergency appendectomy. He states had a few injuries for be a very active kid which include stitches on his forehead from a fall, where he hit his head on the edge of a table and a broken left thumb while playing high school football to which he had surgery to repair.Family history HEALTH HISTORY3Patient mother suffers for bipolar mood disorder and schizophrenia. She also has a history of hypertension and a stroke in the past.
