BP) program through Medicare. As a participant of VBP, PHCMC understands the significance of utilizing strategies throughout the organization to maximize the full benefit of this program. In the current competitive market for health care organizations, there are three areas identified that can be improved to increase reimbursement. This is important for the organization in order to remain viable in the community. The three identified areas for improvement are communication with nurses, sepsis care, and CAUTI rates. HCAHP scores hospital wide for communication with nurses has continuously been below benchmark and strategies need to be implemented to improve this score. The nursing communication total score averages the results of three questions that patients and their families answer in a post discharge survey. The goal is for the nursing communication score to increase by 25% by the end of 2023. In order for PHCMC to maximize reimbursement for this component of the VBP program, nursing communication must be improved. Not only will improved nursing communication benefit the hospital financially, but it will also lead to improved patient education, decreased readmission rates due to increased knowledge of the plan of care, and it supports patient family centered care.
